Bidders MUST review all documents attached to this posting, and complete, sign, and upload the BID FORM and other required forms.
In accordance with Hawaii Revised Statutes § 103-53, vendors must provide a valid tax clearance from the Hawaii Department of Taxation and the U.S. Internal Revenue Service, as a prerequisite to entering into any contract, agreement, or purchase order of $25,000 or more, and prior to releasing a final payment for such purchases. A Certificate of Vendor Compliance that reflects a “Compliant” status from Hawaii Compliance Express (HCE), is acceptable in satisfying the tax clearance requirement. 1) Bidder shall provide information on work plan and approximate project timeline to Kap-CC. All work shall be performed by skilled tradespersons using the best standard practices for each trade. All work shall be performed in accordance with federal wage requirements, as well as the Minimum Wage Rate Schedule and Chapter 104, HRS.
2) Upon award, vendor must provide Certificate of Liability Insurance, listing the Research Corporation of the University of Hawaii, the University of Hawaii, and the State of Hawaii, as Additional Insured. For more information regarding insurance requirements, see Attachment 3.
3) Attachment 1B is provided for Bidder's reference and to establish a basis of design and layout.
4) Bidders are cautioned to read and understand all parts of the terms and conditions contained in the attached General and Special Provisions (Attachments 2 and 3) of the IFB.
5) To be eligible to quote, offerors must possess a valid State of Hawaii Contractor’s License C-25. OR, offeror must possess a General Contractor's License B and any Subcontractors employed must possess the requisite C License(s).
6) All prices are FOB destination, including freight, taxes and all applicable charges. Only firm prices accepted. Final cost cannot exceed quoted cost unless pre-approved in writing via a Contract Amendment. Payment will be via Purchase Order.
7) Contractor shall maintain the work site in equal or better condition when work is completed. Contractor should be aware of any special features, structure, irrigation, fencing, utilities; any damage shall be reported to the RCUH/University immediately and repaired to the University's standards.
8) Access to all buildings and accessible routes must be maintained.
9) Ordering may begin once Contractor receives notice to proceed; onsite work may begin upon receiving notice to proceed and be completed no later than 90 consecutive calendar days following receipt of Notice to Proceed. Work performed on weekends to maintain scheduled timeline shall be at no additional cost to RCUH. |
